Ask ten people whether dairy causes acne and you'll get ten confident, contradictory answers. The research is actually more specific than the shouting suggests, and the specificity is the interesting part: it isn't dairy as a whole that stands out. It's milk, and skim milk in particular.
What the studies found.
The foundational work came from large cohort studies that tracked diet and acne. They found a positive association between milk intake and acne, and, counterintuitively, the association was strongest for skim milk (Adebamowo, 2005). Later meta-analyses pooling tens of thousands of people echoed it: skim milk carried the clearest link, with whole milk and other dairy weaker or inconsistent.
Note the honest caveat baked into all of this: these are observational associations, not proof that milk causes breakouts in any given person. But the signal is consistent enough to take seriously.
The surprise isn't that milk was implicated. It's that skim milk was the worst offender.
Why skim.
The leading explanation runs through hormones. Milk contains and stimulates IGF-1, insulin-like growth factor 1, a signal that promotes the follicular growth and oil production that feed acne. Milk-derived amino acids also spike insulin, which pushes IGF-1 higher still. The reason skim may be worse is that removing the fat concentrates these effects and changes how the milk is processed. Tellingly, people with a rare condition who can't produce IGF-1 essentially don't get acne, which is strong circumstantial evidence for its role.
The practical read.
You don't need to fear a wedge of cheese. But if you're an adult dealing with persistent breakouts, cutting skim milk for a few weeks is a low-risk experiment with real mechanistic logic behind it. And because the whole pathway runs through insulin and IGF-1, it connects to the same metabolic markers that drive adult hormonal acne generally, which are measurable.
